Statutory Health Care Directive Minn. Stat. 145C.16
The State of Minnesota statutes also contain a health care directive that can be used. Here is a link to a fillable PDF copy of the statutory Health Care Directive. Unlike the above two options, this one, passed in 1993, is not required but is instead another option. Hospitals, and those organizations included above offer another form. Any form may be used as long as it meets the statutory requirements.
