Long Term Care Consultation

Consumers applying for Alternative Care or Elderly Waiver services must participate in a Long Term Care Consultation (LTCC). LTCC is an interview by a county public health nurse and/or county social worker with the consumer to assess the person's need for long term care services (in-home, community or nursing home services). One purpose of LTCC is to prevent inappropriate nursing home admissions and promote the use of in-home and community services when appropriate.

Alternative Care

Alternative Care is designed for persons age 65 and older who require a nursing home level of care but choose to receive in-home and community services and, if they would move into a nursing home, would be eligible to participate in Medical Assistance (MA) within 180 days of moving in. MA is a state health care program for persons of limited income & assets.

Financial eligibility for AC as of 7-1-98:

Single person: Income as low as $488 a month and assets up to $24,700 or Income as high as $4,200 a month and 0 assets

Married couple: Combined income about $1,420 a month and combined assets of no more than $55,000

Services offered under AC are:

  • Homemaker services
  • Respite care
  • Adult day care
  • Home health aide
  • Home health nursing
  • Personal care assistant (PCA)
  • RN supervision of PCA
  • Foster care
  • Supplies & equipment
  • Companion services
  • Assisted living services
  • Caregiver training
  • Home delivered meals
  • Transportation
  • Chore services
  • Nutrition services
  • Residential care services
  • Telemedicine devise

Elderly Waiver

Elderly Waiver is similar to Alternative Care except it is for persons age 65 and older who participate in Medical Assistance.

Medical Assistance eligibility as of 7-1-98:

Single person: $487/month income & $3000 assets
Married couple: $583/month income & $6000 assets

Persons with incomes over the limit may be eligible for MA under a "spenddown." A spend down is the amount of income in excess of the monthly income allowed for eligibility. For example, if one's monthly income is $586 but medical expenses (medications, home health care, clinic bills, etc.) exceed $100 a month, the person pays $100 of her income for these medical expenses and MA pays any additional approved medical expenses.

Services under EW are similar as AC except chore services and telemedicine devise are not included. An additional EW service is limited modifications to the home.
