Aging With Dignity's Five Wishes helps people express how they want to be

Five Wishes is used in all 50 states and in countries around the

Honoring Choices Minnesota" is an effort of the
Twin Cities Medical Society and its Foundation
to encourage families and communities to have
discussions regarding end-of-life care choices.
Twin Cities Public Television is supporting
"Honoring Choices Minnesota" with efforts to reach
and engage Minnesota citizens in this effort.
Honoring Choices Minnesota uses the training,
principles and methodology of Respecting Choices
®, a nationally recognized model of Gundersen
Lutheran Health System in La Crosse, WI.
Honoring Choices is available in English, Hmong, Russian, Somali,
and Spanish.
