The Minnesota Health Care Directive Toolkit was developed by a group of professionals with expertise in law, health care, life and death health care decision making, and plain language materials development with the leadership of Marlene S. Stum, PhD, University of Minnesota Extension Service.


 


Aging With Dignity's Five Wishes

 

Aging With Dignity's Five Wishes helps people express how they want to be treated if they are seriously ill and unable to speak for themselves. It deals with all of a person's needs: medical, personal, emotional and spiritual. Five Wishes is available in print, online, and in 26 languages and Braille. Five Wishes is used in all 50 states and in countries around the world. It meets the legal requirements for an advance directive in 42 states, one of which is MN. Five Wishes states.

 

 

 

Honoring Choices Minnesota

Honoring Choices Minnesota" is an effort of the Twin Cities Medical Society and its Foundation to encourage families and communities to have discussions regarding end-of-life care choices. Twin Cities Public Television is supporting "Honoring Choices Minnesota" with efforts to reach and engage Minnesota citizens in this effort.

Honoring Choices Minnesota uses the training, principles and methodology of Respecting Choices ®, a nationally recognized model of Gundersen Lutheran Health System in La Crosse, WI.
