The Minnesota Department of Human Services and the Minnesota Board on Aging are seeking nominations to honor outstanding organizations and/or individuals who work in the aging and/or disabilities community. Awards winners will be showcased at the Monday evening, August 18, 2008 social/networking gathering. Please join us in the recognition of the outstanding accomplishments of this year’s awards winners as we present:
This award recognizes an organization or individual that has demonstrated public policy leadership that has resulted in improving the quality of life for elders, the people with disabilities and their communities.
This award recognizes an organization or individual that has championed innovations in direct services, coordination of services, community-based partnerships and increased the resilience, interdependence and quality of life for elders, the people with disabilities and their communities.
This award recognizes an organization or individual that has promoted exemplary practices, services and/or advocacy for elders, the people with disabilities and their communities.
This award recognizes an organization or individual that has improved policies, services or quality of life practices for elders, the people with disabilities and their communities.
A panel of diverse representatives in the fields of aging and disabilities will make award determinations.
