Family caregivers can find information from a number of books. ‘A Family Caregiver’s Guide to Hospital Discharge Planning,’ a free booklet designed to help families prepare for the daunting task of caring for a disabled or chronically ill loved one, available through the National Alliance for Caregiving at www.caregiver.org or the United Hospital Fund at www.uhfnyc.org.
